Clinical Pharmacist
➡️Job Description:
📌Review patients’ medical histories, laboratory results, and other relevant information to identify drug-related problems, contraindications, and potential interactions.
📌Monitor patients’ response to medications, assess therapeutic outcomes, and make adjustments as necessary to achieve optimal therapeutic outcomes.
📌Provide drug information and clinical guidance to healthcare professionals to promote safe medication practices and prevent medication errors.
participates in writing or evaluating Medication therapy protocols, policies and procedures.
📌Provide guidance on lifestyle modifications, drug interactions, and potential drug-food interactions to optimize medication outcomes.
Developing a pharmaceutical care plan.
➡️Job Requirements:
📍Bachelor’s degree in pharmacy.
📍(PharmD) or BCPS degree (added advantage).
📍 2 – 4 years as clinical pharmacist .
📍Strong clinical knowledge and understanding of pharmacotherapy principles, drug interactions, and medication safety practices.
📍Attention to detail and accuracy in medication order review, dosage calculations, and documentation.
